Psychological disorders, such as depression, bipolar, and anxiety disorders can present several complications for patients of all ages. These disorders affect patients physically and emotionally, potentially impacting judgment, school and/or job performance, and relationships with family and friends. A 70-year-old male Hispanic American was referred to a specialist by his health care physician as part of standard testing to rule out an organic cause for his sadness. The patient received a score of 51 on the Montgomery- Asberg Depression Rating Scale (MADRS), which denotes that the patient is experiencing severe depression. The patient no longer enjoys his routine activities, and the depressive symptoms are impairing his ability to concentrate at work.

 In this scenario the beginning the patient on Zoloft 25mg has the benefit of reducing the patient’s depression by 25% when compared on the MADRS, but at the cost of the patient suffering from erectile dysfunction (ED) on of the side effects of Zoloft. Adverse reactions such as ED in patients with depression is problematic due to the psychological impact that ED can play in a patient whose self-esteem is already diminished. Medication changes need to be in line with what the patient wants and is willing to experience; they must be made aware of the complications and adverse effects before being switched to a different medication or dose.
