The change model I am proposing is based on my operational observations of a post-acute nursing facility, is the over dependence of antibiotic products, of which I believe is contributing to the proliferation of antibiotic resistant organisms. Additionally, I would argue that the proverbial cat is out of the bag, in terms of the amount of drug resistant microorganisms currently understood, as there should be major steps taken to mitigate the proliferation of new strains of these types of organisms.
Complex and chronic wounds are often treated with antibiotic products and in some instances wound healing may be stalled by reinfection, thus creating a need to treat the patient with additional antibiotic products. Two models that may help my evidenced based practice proposal is the Iowa Model of Evidence-Based Practice and the Kotter and Cohen’s Model of Change. The Iowa Model of Evidence-Based Practice (IM-EBP) benefits from a recent revision in which the main framework of the model consisting of three decision points were refined to allow small units within large organizations to operate (Buckwalter et al., 2017).
In Kotter and Cohen’s Model of Change there are eight steps within the framework that are pinned by the overall guiding principle of the model which is focusing on the emotional and lived experiences than regurgitating facts on why change should occur (Melnyk & Fineout-Overholt 2018). Of both models discussed above, the Iowa Model of Evidence Based Practice would facilitate change at two levels including upper organizational levels, and local levels.
The Iowa Evidenced Based Practice model has been utilized to develop and deploy process changes within clinical practice settings within a local organization that may translate to organizational wide implications. An example of the IM-EBP includes the development of procedural guidelines in the administration of hyperosmolar 3% NaCl solutions via peripherally intravenous access versus, central venous access, as both venous access points have inherent risks yet peripheral administration is prone to less lethal complications (Janotta et al., 2021).
In the IM-EBP mentioned above, the framework consisting of five steps including step 2 which is establishing an interdisciplinary team, integrated input from committees such as Neurocritical Care quality improvement, Clinical Nursing, Intravenous Therapy Nursing, and Pharmacy (Janotta et al., 2021). In my own evidence-based practice project, I would integrate similar committees into the disciplinary team, for instance Pharmacy services, Wound Treatment Specialists, Clinical Nursing Operations.

When looking at different models and frameworks for facilitating change, it is important to know the characteristics of each and which ones would be the best fit. Two examples of models used to facilitate change in the healthcare setting include the Iowa Model “Revised” Evidence-Based Practice to Promotor Excellence in Health Care (Iowa Model) or the Advancing Research and Clinical practice through close Collaboration (ARCC) Model. These two models can be used for systematic change.
The process of taking evidence from research and implementing it can be daunting without solid guidance. The Iowa Model and ARCC model have been used by many researchers around the world. The Iowa Model uses step-by-step processes with feedback loops (Tucker, et al. 2021). Step-by-step processes help researchers plan how they will implement research into clinical changes. The Iowa Model also uses a team-based approach which is beneficial to implementation in the clinical setting where other team members can give input and help guide decisions for future needs.
The ARCC model relies on the assessment of the facility’s culture and readiness for change which is the strength of this model (Tucker, et al. 2021). This model is not as easy to implement as the Iowa model which has a step-by-step road map to follow.
When comparing the two models, I feel that the Iowa Model would be the best model to use for my EBP project because of the step-by-step process that is easy to follow. The Iowa Model was found to be very easy to use because of its easy-to-follow nature and very straightforward (Buckwalter, et al. 2017). A study in 2017 stated that 68.4% of the participants found it easy to follow. Having a user friendly model for EBP implementation can help clinicians in the field who are very busy.

According to Melnyk and Fineout-Overholt (2018), models are developed to organize our perception of evidence-based practice and to understand how the different aspects of EBP related to each other to provide improved care and results. There are eight models created to guide the incorporation of EBP to improve change. The Stetler Model is also called a practitioner-oriented model due to its emphasis on critical thinking and utilization of finding by individual practitioners (Kim, 1999; Stetler & Marram, 1976, as cited in Melnyk & Fineout-Overholet, 2018).
The Stetler model has five phases. Phase I (Preparation) identifies the priority need and the EBP project purpose. Phase II (Validation) determines the level and quality of sources of evidence. Phase III (Comparative Evaluation/ Design Making) summarizes the evidence results and evaluated the similarities and differences of sources of evidence. Phase IV (Translation/ Application) identifies practice indication that justifies the implementation of findings for change. Phase V (Evaluation) determines the expected results and assesses if the goal of EBP has been met or not (Springer Publishing Connect, n.d.).
The other model is the Johns Hopkins Nursing Evidence-Based Practice (JHNEBP). According to Springer Publishing Connect (n.d.), this method applies a problem-solving approach to clinical decision-making. This model is used to meet the needs of nurses in direct care. There are three steps which are practice question, evidence, and translation. Practice question is when the EBP question is identified. Evidence is when the research team describes the quality of evidence and strength of the evidence. The translation is when the action plan is made, change is implemented, and evaluation.
In my evidence-based practice proposal, I believe the use of Johns Hopkins Nursing Evidence-Based practice is helpful because the said method focuses on the direct care of nurses. In relation to that, my proposal focuses on providing practice changes to make patients comfortable and increase quality care after the surgery. Additionally, my proposal started with practicing questioning on how to lessen the use of opioids/ narcotics to the patient after surgery while still will be able to provide quality care. According to Wyant (2017), the John Hopkins model is clinician-focused and allows fast and suitable application of current research and best practices. My proposal is guided by a variety of current studies and research that supports the positive impact of the proposed project on the improvement of quality patient care.
