In healthcare, an interdisciplinary team collaborates to achieve the desired outcome or goal for the patients. An interdisciplinary team involves professional healthcare team members from different disciplines who work together. Using a multidisciplinary team approach basically involves team members from different disciplines, together with the patient to create a plan of care for the patient (Health, 2021). This creates discipline-specific plans that are implemented simultaneously to achieve the best possible outcomes. With this said, the characteristics of a good working interdisciplinary team is the appropriate skill mix that supports an environment that supports a team, great strategies to communicate throughout the team, and positive results.
Examples of a successful interdisciplinary team would be a palliative care RN working with a nursing home team along with a patients family who is critically ill and has a very poor prognosis. The palliative RN can communicate with the family and walk them through many options. Also, the palliative RN can provide resources and guidance with complex decision-making. Interdisciplinary teams are essential in the care of patients with threatening illness and their families (Science Direct, 2020).
On the other hand, patients who need dialysis who are hospitalized have a dialysis RN who is employed by outside companies to provide inpatient dialysis. This RN works collaboratively with the healthcare team as an interdisciplinary team member to provide dialysis services while the patient is hospitalized. This example is a dynamic process that involves two or more health professionals with complementary backgrounds and skills, sharing common goals (Nancarrow et al., 2013).
I have experienced both positive and negative characteristic when working in a team environment. One positive example, was when I was working as a dialysis nurse in a clinic, we had the team environment and worked collaboratively with the dietitian when patients had questions regarding the strict diet that dialysis patients are on. The dietitians had many recipes for patients and gave many resources of different dietary foods that patients could consume without affecting their diet.
These valuable resources and team working collaboratively helped achieve desired outcomes for patients by simply following their diet. On the other hand, I have had negative characteristics working in a team environment, recently, working with a member of social services who lead a team of 6 and would not hold their respective team accountable. A patient was discharged without the proper discharge criteria and instead of bringing the information back to the team, the leader of social services blamed the patient, doctor, and everyone else. Thus, this had a negative outcome of the patient and really lowered the team’s moral of working together.

And also, I have experienced a lot of positive rather than negative results with working interdisciplinary. As a clinical manager for a home health agency, I supervise multi disciplines in my team from nursing, physical, occupational & speech therapy, medical social worker and home health aide. For example, a home health referral for a recent CVA patient and is discharged to home, RN will open the case and depending on the referral of the doctor for other disciplines they will follow for evaluation after.
If during the assessment of the RN, that patient can benefit from Physical therapy to establish HEP and occupational therapy due to hemiplegia, RN will get orders from PCP to have both PT/OT to evaluate and provide their own plan of care. Every discipline has its own goals, each discipline collaborates with each other to achieve the goals stated. One of the challenges with multi disciplines with a home health patient, is scheduling the time for the visits, most patients does not want PT/OT on the same day, since it would be too tiring for them.
With interdisciplinary teamwork especially in home health, it’s very challenging to meet in person to discuss the care plan, especially if the geographic area of patient care is wide. However, our agency makes sure we have our weekly IDT’s attended by all disciplines including our medical director to discuss the updates on the case, this is via video meetings. As Wolters Kluwer (2018), had mentioned, that defining the objectives of interdisciplinary meetings, along with specific patient goals and intended outcomes, is essential for creating a plan of care that meets the needs of the patient while incorporating feedback from the entire multidisciplinary team.
