Nola Pender’s Health Promotion Model is a commonly used model for changing unhealthy behaviors and improving health. It has been verified for its efficacy in changing unhealthy habits and is very successful (Bijani et al., 2022). This model uses behavior-specific cognitions and encourages health promoting behaviors, characteristics, enhances self-efficacy, corrects behaviors, and improves communication (Bijani et al., 2022). The areas it focuses on is the individual characteristics and the experiences the patient has been through, behavior-specific cognitions and behavioral outcomes.
This model is ideal for patient care because it focuses on the individual and targets specific behaviors. For example, a patient admitted with a COPD exacerbation. The personal factors: patient is a smoker and overweight. The immediate behavior to change is smoking. The behavior specific cognitions would be their perceived benefits of quitting, the action of how to quit, perceived self-efficacy and influences that could potentially prevent them from achieving goal (ex: wife smokes). As a nurse we would come up with a plan of action and health promoting behaviors in order to achieve the behavioral outcome. This model takes into consideration environmental, socioeconomic, and situational influences so I think that is a realistic model and I do not see many cons. However, the patient would have to be agreeable to change.
Kurt Lewin invented The Change Theory of Nursing. It is a three-stage model known as unfreezing-change-refreeze model that requires past learning needs to be rejected and then replaced with a new behavior (McFarlan et al., 2022). The unfreeze phase is determining what needs to be changed, ensuring support, and creating the need for change. The change phase is to communicate, empower, and involve people in the process. The refreezing phase is when changes are implemented and monitored and can be adjusted as needed until the behavior becomes permanent.
With my example above to quit smoking this theory would need to make sure there is support before starting the activity and understanding and managing the expectations and concerns from the patient. During the change process communicating often with the patient and involving other people for support. In the last phase, celebrating success and developing a way to sustain the change. This model I noticed that taking into consideration the environmental and situational aspects of a patient’s life was not addressed.
Out of these two theories I would incorporate Pender’s model in practice because I think it would benefits patients in the hospital setting. The change theory relies on frequent communication and support and in the hospital setting we often do not have patients for long periods of time. With the Health Promotion theory, we are setting them up for success with an action plan and also considering the entire factors that will go into this change.

Virginia Henderson created the Nursing Need theory which focused on the need to promote independence of the patient to improve their over healthcare goals (Tourville & Ingalls, 2003). This theory focuses on how the nurse can facilitate the patient in meeting their basic human needs as it pertains to healthcare. The pro to integrating this theory into my future practice as a nursing leader is that it will make the patient’s more compliant with their medical needs. By putting the power back in the hands of the patient’s (provided they are assisted, educated, and influenced by the nurses) I believe it increases patient satisfaction and compliance. The con to integrating this theory into my future nursing practice as a nurse leader is that this theory is heavily reliant upon the nurse providing the patient with adequate information, cheerleading the patient to making the right choice, and using their own creativity to individualize patient care.
Unfortunately, not all nurses are equal, and some nurses do not belong in the profession. If the patient, receives care from this type of nurse, the patient will not be supported adequately to make good health decisions. Finally, Henderson’s theory brings up the ethical issue of a patient’s right to autonomy. For most patient’s this will not be a problem, as majority are within sound mind to make their own decisions. However, for the patients who are not of sound mind, ethically it is wrong to increase their independence by letting them make all of their own medical decisions.
Joyce Travelbee created the Human-to-Human Relationship model which empathized the importance of nurses in creating hope for the patient (Petiprin, 2020). The nurse and patient, family, and community relationships were key to driving this sense of hope particularly in a time of healthcare struggles and suffering. This theory extended upon the interpersonal relationship models of Orlando and Peplau.
Travelbee’s theory relates to patient care in that it describes the pivotal point of fostering nurse to patient, nurse to family, and nurse to community relationships in creating a hopeful attitude within the not so pleasant times of having a sick loved one or being sick in the hospital. This theory can incite behavioral change in that the patient is motivated by the nurse to advocate for their health. The nurse, incorporating the whole patient (including their family and community), creates an individualized plan to help the patient with their health.
As a nursing leader, the pro to using this theory is that it fosters positive relationships between the nurse and the patient. This makes the patient more likely to be compliant within their healthcare. The con to using this theory is that it is dependent the motivation and professionalism of the nurse. If the nurse is not happy within their profession and/or has no motivation to help the patient, this could create a dissatisfaction and disconnection for all parties and a lack of compliance when it comes to patient care. Finally, there are no ethical issues in integrating this theory within my future nursing practice.
