I chose to discuss Marilyn Anne Ray’s Theory of Bureaucratic Caring as I feel it most relates to healthcare quality and patient safety. Ray’s is a middle-range theory that focuses on how nursing interacts with the healthcare organization as a whole and how nurses must have knowledge and understanding of the organization as a bureaucracy and how the organization works together as a caring system.

All the parts of the organization and the whole of the organization are one in the same, working together coherently, even though the different parts may be working at different paces on different efforts. Ray’s theory focuses on the patterns of caring within the relationship between the nurse, patient, and administrator within the bureaucratic context of the organization. Spiritual-ethical caring is connected by qualitative processes: physical, sociocultural, technological, educational, legal, political, and economical (Smith, 2020). 

As a nurse working in quality and patient safety, I am frequently interacting with and supporting the efforts of the organizational administration, all with the same goal of providing high-quality care and supporting a culture of patient safety. An important part of improving quality and safety is supporting the different parts of the organization and the organization as a whole in making changes in processes and systems.

Smith (2020) stresses the importance of Ray’s theory in making needed changes for improvement. Rather than continuing approaches of predictability and control that may have worked in the past, we must be open to new understanding. Nurses and other professionals within the organization need to be open to change for the benefit of improving the quality of care provided and improving patient safety (Smith, 2020).

Hildegard Peplau’s Interpersonal Relations Theory focuses on the relationship between the nurse and the client as the foundation of the nursing profession. Peplau formed an interpersonal model that emphasized the need for the nurse and the client to have a give and take relationship as opposed to the passive receiving of treatment of the client and the following of orders by the nurse. The theory has four components: person, environment, health, and nursing. The client is a person with a need. Health is the forward, ongoing movement toward productive living.

The environment considers society and the patient’s culture when adjusting to care routines. The nursing component is defined as a specially educated nurse who can recognize and respond to the health needs of the client. The therapeutic nurse-client relationship involves two or more individuals interacting toward a common goal. The Interpersonal Theory and nursing process focuses on the nurse and client working together by utilizing problem solving techniques to meet the needs of the patient (Gonzalo, 2021).

Peplau’s theory focuses solely on the relationship between the nurse and patient, and how the patient and nurse work together toward common goals, whereas Ray’s theory focuses on how the nurse and administration of the organization work together with the common goals of meeting the patient’s needs. The nurse is at the foundation of both theories, and both theories describe how the nurse interacts and forms relationships with either the patient or the patient and the organization.

The two theories I chose are; Peplau‘s Theory, and Neuman in 1972. Peplau’s theory intent is to focus on the important relationship between the nurse and the patient, and that nurses can be an important part of a patient’s environment for better health. Neuman’s theory intent is to focus on stress reduction as being an important part of the nursing care plan and practice (Wayne, 2021).

Peplau’s theory aims for nurses to provide needed survives to patients and be aware of their needs at all times. Patient’s need to be respected, heard for, and cared for with empathy and dignity. Nurses need to be professional, and care for patients with vigilance (vigilant guardians). Neuman’s theory suggests that health and wellness is the optimal stability of human beings, and the same applies to patients when they are sick in the hospital. They lack their optimal stability, and it is part of the nurses’ job to restore and maintain stability and health through reducing stress. The theory suggests that stressors can be within a person (intrapersonal), between individuals (interpersonal), or outside an individual (extrapersonal). Neuman’s theory has been used in professions other than nursing as well. (Tourville & Ingalls, 2003).

I think Peplau’s theory is and will be more relevant to my future and any health care provider. Clinicians need to care for patients with dignity, respect and professionalism. Patients have needs and requirements because they are at their vulnerable state. Their health, the most important component of life, is at risk. They are not at their best ability to care for themselves or even make any decisions (let alone medical decisions). I think that any person under stress and instability isn’t able to think clearly or make decisions. Nurses’ job is to advocate for patients, and be their vigilant guardian while sick and vulnerable.

Nursing theories are designed to describe nursing care and provide the nurse with a structure for all decisions related to patient care (Tourville & Ingalls, 2003). There are many different nursing theories dating all the way back to the 1800s. Florence Nightingale was the pioneer of modern nursing and the creator of the Environmental Theory.

Florence’s theory defined five items in the environment that were crucial to a patient’s care. These five things included pure water, fresh air, direct sunlight, efficient drainage, and sanitation. By using these five things, a nurse could aid a patient’s disease process and help facilitate a faster recovery. In the most recent times, Kristen Swanson created another nursing theory called the Theory of Caring (McKelvey, 2018). 

According to Kristen, nursing is defined by caring for others and their well-being. By caring for them, the nurse relates to the patient with whom they are responsible for. The nurse shows a nurturing sense of self to the patient. The theory of caring offers a theoretical framework for advancing in evidenced-based research, education, and refining up-to-date practices. Finally, this theory brings nursing back to its traditional core values of caring and healing others. 

Both the Theory of Caring and the Environmental Theory are alike in that they are nursing theories created by experts in the health care field (McKelvey, 2018; Tourville & Ingalls, 2003). Also, both theories are categorized as grand theories. They are broad in concept and can be applied to all populations, departments, and disease processes. Finally, both theories are alike in that they focus on improvement in patient care and recognize the nurse as the center of this process. On the other hand, both theories differ.

The Environmental Theory focuses on tangible things that can improve patient care (i.e. sunlight). The Theory of Caring focuses on intangible items that improve patient care (i.e. the nurses ability to show a caring nature to the patient). Also, the theories differ in terms of time relevance. At the time Florence created the Environmental Theory, things like sanitation and clean water were not common. Today, in the United States, majority of hospitals have clean water, proper sanitation, and windows in every hospital room (recognizing the importance of sun light on a patient’s recovery process). Post-pandemic, I have seen a huge shift in nursing burnout. The Theory of Caring is something that is extremely relevant today because of Covid-19. 

As a future nursing leader, I feel that Kristen Swanson’s Theory of Caring is most relevant for my professional role (McKelvey, 2018; Tourville & Ingalls, 2003). A caring manner is an essential tool that makes one a great nurse. Having a caring attitude is not something that can be taught. It comes from within and sparks a desire to help others. I truly believe that anyone can be taught anatomy and physiology.

Anyone can give meds and notify doctors. What sets nursing aside from all other healthcare professions, is our caring nature and our drive to help the patient. Our caring nature motivates us to not only pass the medications or help with procedures, but our caring nature moves us to treat the entire patient. It pushes us to fix the patient physical, mental, and emotional while offering a loving hand. As a nursing leader, I want to exemplify a caring nature to not only the patients, but also my staff.
