PICOT:

For patients who underwent laminectomies, spinal decompression, discectomies, and spinal fusions at an academic medical center, what is the impact of implementing an early mobility protocol on post-operative neurosurgical spine patients, compared to the current practice of decreasing the length of stay in 8-10 weeks?

TRANSLATIONAL MODEL

The evidence-based project I have chosen, which focuses on early ambulation and reducing the length of stay for post-operative spine patients, has significant practical implications. As discussed in our previous courses, using translational models or change frameworks is essential for successfully executing this scholarly project. I’ve decided on the knowledge-to-action (KTA) framework in this context. This systematic framework involves identifying and validating existing knowledge, adapting it to current practice, and making it a suitable choice for our evidence-based intervention (Torres et al., 2023).

The KTA framework has six stages that can be effectively discussed during implementation. The first stage is identifying the problem and the practice gap. As discussed, the length of stay of our post-operative spine patients has increased, impacting their outcomes and utilization of hospital resources. The second stage determines whether it can be applied to the current practicum site, following a shared decision among our stakeholders.

The third stage identifies the barriers and challenges that may be encountered during the implementation process. This also means plans to mitigate and overcome these barriers. The fourth stage is implementing the evidence-based intervention at the practicum site. Phases five and six are the conclusion and analysis of the intervention. Phase seven involves sustaining the project by embedding it into the practice and culture of the practicum site. Involving stakeholders throughout the project process ensures a sense of ownership and leadership among them (Ten Ham-Baloyi, 2022).

I have discussed the framework above, but how will we incorporate it in the coming weeks?

Week 1: This includes phases 1 and 2, which identify the problem and involve meetings with stakeholders to assess the importance and necessity of the practice change.

Week 2 – 9:  Phases 3 and 4 include identifying the challenges and barriers that the project may face. This also means involving stakeholders and identifying champions to ensure the dissemination of practice change.

Week 9: Phases 5 and 6 ensure the analysis and conclusion of the project. This also includes presenting data to stakeholders and unit champions.

Week 10: In phase 7, we sustain and embed the knowledge into the culture and practice.
