The purpose of the project at the Desai Medical Group is to address the prevalent challenge of managing anxiety in adult psychiatric patients. Anxiety continues to be a significant concern in the facility and hurts their quality of life (Penninx et al., 2021). PICOT:

For patients who underwent laminectomies, spinal decompression, discectomies, and spinal fusions at Cedars Sinai Medical Center, what is the impact of implementing an early mobility protocol on post-operative neurosurgical spine patients, compared to the current practice of decreasing length of stay in 8-10 weeks?

Project Purpose:

This DNP project aims to enhance patient and operational outcomes for patients undergoing spine surgery at the medical center. The executive leadership group, led by the Chief Operating Officer, has released the organization’s priorities for leaders to improve and explore. One of these priorities was the need for patients to mobilize during their hospital stays and in the early post-surgical period, thereby decreasing the length of stay.

Additionally, spine value-based coordinators have discussed and reported an increasing length of stay among scheduled procedures. Early ambulation after spine surgery improves patient outcomes, prevents hospital-acquired complications, and decreases the length of stay. Notably, a nurse-initiated program in early ambulation of patients has significantly impacted outcomes by reducing the length of stay and empowering nurses to encourage and serve as champions for early ambulation.

Project Aims/Objectives:

This project aims to decrease the length of stay among post-operative spine patients through a nurse-initiated program in early ambulation. The objectives of this DNP project are to 1) educate nurses on the early ambulation protocol with their criteria, 2) utilize other resources that can assist in the implementation of early ambulation, 3) increase patient and nurse satisfaction with those participating in the early ambulation protocol.

Understanding the importance of early ambulation is just the beginning. Influencing and changing behaviors for both patients and nurses can impact outcomes and be embedded in practice and culture.
