Organizational Assessment
Health organizations play a crucial societal role by providing essential medical services, promoting well-being, and addressing health-related challenges. However, even the most well-intentioned organizations can face problems and gaps hindering their effectiveness and impact. According to Miake-Lye et al. (2020), an organizational assessment is essential in identifying these issues and implementing solutions to improve performance.
I have identified staff burnout and compassion fatigue as the primary organizational issues facing the mental health organization where I intend to conduct my DNP project. This paper aims to perform an organizational assessment to identify gaps, outline steps to examine the assessment process, establish DNP project goals, and align them with the organization’s mission and vision.
The comprehensive assessment process will employ various methods to identify organizational gaps related to staff burnout and compassion fatigue. Initial steps will involve direct observation and informal discussions with frontline staff, nursing leaders, and relevant employees, offering qualitative insights into daily challenges and perceptions. Structured surveys will provide quantitative data on burnout indicators, ensuring representation across all staff levels.
Involving the entire staff is crucial for a holistic understanding of the issue (Nilsen et al., 2020). Stakeholders encompass executive leadership, nursing managers, direct care providers, patients, and their families, offering distinct insights ranging from financial implications to the impact on patient care. Engagement via focus groups and surveys will enable a holistic understanding and collaborative identification of improvement opportunities.
My DNP project aims to reduce staff burnout and compassion fatigue, thereby enhancing the quality of care provided to patients with mental health and psychiatric needs. This will be achieved through implementing evidence-based interventions, organizational policy changes, and fostering a culture of well-being (Aryankhesal et al., 2019). Ultimately, the project aims to create a sustainable work environment that promotes staff resilience, engagement, and emotional well-being.

Sustainable outcomes improve intervention projects, and it is crucial to prepare with data, be community-centered, and have a plan to communicate progress often (Brakema et al., 2021). I do not work for the acute care hospital where I plan to conduct my project. Therefore, the process for opportunities for improvement comes directly through my preceptor and primary stakeholder, the Interim Chief Nursing Officer (I-CNO). Introductions were made with my I-CNO in September 2023. I aim to continue working on global burden of disease projects related to patient safety, activities of daily living, and prevention (Bernabe et al., 2020).
Namely, oral care should be performed two to four times daily, or the proper use of an incentive spirometer, to prevent hospital-acquired pneumonia. Intervention projects need relational leadership (Cummings et al., 2021). My preceptor encouraged me to pursue something I am passionate about, and in December 2023, provided some data indicating that the hospital had not met its pneumonia goals for 2023. Other stakeholders include the education and library team members for my access to the hospital, members of the internal review board, technology support, and the hospital statistician.
The DNP project aims to decrease incidents of non-ventilator hospital-acquired pneumonia. The intervention promotes an oral hygiene protocol, including brushing teeth two to four times daily for 120 seconds, for all admitted patients on a medical-surgical floor. The technology and educational teams will be necessary to evaluate pre- and post-project implementation medical records. Evaluation will include documentation of pneumonia diagnosis post-admission, return admissions for pneumonia within 30 days, and direct care staff documentation of oral hygiene.
The hospital’s mission, “to improve the health and well-being of those we serve,” and its vision, “to be the partner of choice as we transform healthcare for our communities” (HonorHealth, 2024, para 1,2), are directly aligned with the project’s goals to decrease non-ventilator hospital-acquired pneumonia. This alignment is significant as it underscores the project’s strategic importance and its potential to contribute to the hospital’s overall mission. The project is transformative in that it addresses primary and expected care that can have significant outcomes in lowering pneumonia rates.
