Early ambulation was identified as a critical organizational need and was given high importance. During a progression of care rounds (POCR) with the interdisciplinary team, we identified one significant barrier to our post-operative spine patients’ discharge: the delay in mobilization and awaiting physical therapy. During meaningful leadership rounds, patients have also expressed frustration with waiting for the physical therapist to arrive. We then asked, “Why can’t the nurse ambulate the patient?”.
Rupich et al. (2018) have explored a nurse protocol to ambulate patients early, which can impact patient outcomes and, in turn, decrease the length of stay. Part of the Enhanced Recovery After Surgery (ERAS) protocol identified three phases of care for perioperative spine patients and determined that early ambulation after surgery for spine patients improved clinical outcomes and decreased the impact of postoperative complications (Debono et al., 2021). This has been supported by Kim & Mobbs (2021), who systematically reviewed ten articles on early ambulation for post-operative spine patients, noting that these articles share themes related to decreased length of stay. The evidence-based intervention will address the organization’s problem and achieve its goal for fiscal year 2024.
