The patient complains of increased fatigue and increased irritability, and tearfulness. She is unhappy and denies suicidal ideation. Other tests are necessary to rule out possible causes of her presentation. A complete blood count is within normal limits, ruling out an infection or anemia as a probable cause of fatigue and lack of energy. The thyroid function tests are also within the normal limits, which rule out thyroid disorders such as hypothyroidism that may present with mood changes (Park & Zarate, 2019). Normal thyroid studies also rule out hypothyroidism as a possible cause of obesity. Drug tests are negative for all drugs. Results from the history indicate that Angela is most probably suffering from adolescent depression. Normal laboratory tests rule out substance use or a medical condition as a possible cause of her presentation.

The expected tests would be a complete blood count to rule out anemia or infections. Thyroid function tests to rule out thyroid disorders.  Fasting blood sugar to check Angela’s sugar levels. Her glycated hemoglobin levels (HbA1C) are used to check how well her blood sugars have been controlled over the last few months. Lipid panel to check for cholesterol and triglyceride levels.  Lipid abnormalities are prevalent in patients with obesity (Pirillo et al., 2021). A urine cortisol test to rule out possible causes of obesity.

Patients with obesity usually have high levels of cortisol. High cortisol levels can also increase the appetite and cause adipose tissue to redistribute in the abdominal region. AC is suffering from adolescent depression. She also has obesity. She is at risk of developing diabetes because of her obesity and a positive family history of diabetes. Drug tests are negative and this rules out substance-induced depressive disorder, usually characterized by depression secondary to consumption or inhalation of a substance.
